ADA Recognized Program

Registered nursed and dietitian (CDEs) provide comprehensive training to adults with Type 1 and Type 2 diabetes as well as gestational diabetes and pre-diabetes. Includes an individual consultation with RN and RD, and 4 small group classes (meet weekly). Other services include Nurse Practitioner CDE visits to evaluate and manage insulin and insulin pumps for improved control. Most insurance accepted. Referrals from PCP and preregistration or appointments are required for class and services. Support group available.

ThedaCare Diabetes Education Program

A diabetes self-management program for those newly diagnosed with diabetes or people who have had diabetes for years. Gain the information you need to manage your diabetes.

A Diabetes Self-Management Program accredited by the American Diabetes Association consisting of classes and individual sessions. Services also include medication and insulin pump management as well as continuous glucose monitoring systems (CGMS). Dietitians and nurses are available; most are Certified Diabetes Educators.

Gestational Diabetes education is also a service provided by a Certified Diabetes Educator at both Appleton Medical Center and ThedaClark.

All insurances accepted including medicare and medicaid.

Many locations available: ThedaClark in Neenah, Shawano, New London, Appleton Medical Center, Oshkosh and Fox Cities Community Clinic. Hours typically 8 a.m. to 5 p.m. Classes are available at varying times from 9 a.m. to 6 p.m. depending on location.

A physician referral is required. Referral can be faxed to (920) 831-5093 (Appleton Medical Center) or (920) 729-2921 (Theda Clark).

Diabetes Education for those newly diagnosed or previously diagnosed with diabetes.

We are an American Diabetes Association approved diabetes self-management education program and provide diabetes self-management education as an interactive, ongoing process that involves the person with diabetes and the educator/diabetes coach. The intervention aims to achieve optimal health status, better quality of life and reduce the need for costly health care. We focus on seven things patients can do that are essential for improved health status and greater quality of life: healthy eating, being active, monitoring of blood sugars, taking medication (if needed), problem solving, healthy coping and reducing risks. Our diabetes educators give people with diabetes the knowledge, skills and tools they need to successfully manage their diabetes and avoid many of the complications associated with the disease.

We provide our services Monday through Thursday, from 8:30 a.m. to 4:00 p.m.

Our two diabetes educators are certified diabetes educators with a Master's Degrees; one a Registered Nurse and one a Registered Dietitian. Our services are covered by Medicare and most commercial insurance plans. However, individuals should check with their specific plan about coverage and if a referral is required.

Chesapeake Regional Medical Center provides complete diabetes care for adults and children. From inpatient care to support groups, you'll find a broad range of services here. Whatever your need may be, the members of the Chesapeake Regional Medical Center family can help.

Diabetes/nutrition services at the Lifestyle Center
The diabetes/nutrition program at Chesapeake Regional Medical Center's Lifestyle Center has received national recognition by the American Diabetes Association for being a quality diabetes education program since 1991. To help you learn how to manage your diabetes, we offer individual counseling with certified diabetes nurse educators and dietitians, group diabetes education classes called "Diabetes University" and carbohydrate counting classes. To enroll, your physician must fax or call a referral, or you may contact us yourself. Participation will require a referral from your physician and this program is usually covered by insurance. Cooking classes using our full teaching kitchen helps you learn to cook delicious foods that meet your individual health goals, including diabetes, hypertension, high cholesterol and weight control. We also offer diabetes support groups and camps for children with diabetes.

Inpatient care
If you need inpatient care, you won't find a more knowledgeable, dedicated staff than the specialists in our Diabetes Unit. Diabetes educators, dietitians and other health care professionals will answer your questions and help you control and manage your diabetes. Even if you have a special condition that requires treatment in a different unit of the hospital, you can rely on our expert diabetes care. Our staff work closely with all diabetes patients, guiding you every step of the way.

Home health services
People who are homebound or recuperating from a recent hospital stay may need home health services. Chesapeake Regional Medical Center's Home Health is the solution. With a diabetes educator on staff, Home Care Services can provide expert care for patients with diabetes. Most insurance policies cover this service. Special programs for children
We provide a variety of special programs for children with diabetes. Camps and classes staffed by diabetes specialists offer a unique opportunity to foster learning and independence for children and parents. Chesapeake Regional Medical Center also offers support groups and many other special programs for parents and children.

Gestational diabetes
Finding out that you have gestational diabetes can be frightening. We can help you manage diabetes during your pregnancy to help insure the health of your baby. You will learn about proper diet and nutrition, and controlling your blood sugars. We will provide you with a loaner meter and guide you in home glucose monitoring.
