Southwest Health Center Diabetes HEALTH Program

Diabetes Self Management Education Classes for persons with type 1, type 2, or gestational diabetes, including Insulin Pump Therapy

ADA Recognized diabetes self-management classes taught by RN and RD Certified Diabetes Educators. A provider referral is required and we accept all insurances. Financial assistance may be available. Initial classes are taught individually for up to ten hours in divided sessions. Annual review classes are taught in a group setting. Classes are appropriate for all age groups and are scheuled Monday-Friday at times convenient to participants.
