Howard University Hospital Diabetes Self-Management Program

American Diabetes Association recognized program for meeting the national standards for diabetes education

The program is taught by a registered dietitian and registered nurse who are Certified Diabetes Educators. Patients will enhance their knowledge on proper food selection and improved diabetes self-management skills Patients of all ages with either type 1, type 2 or gestational diabetes may be referred. Group diabetes education classes are held once a month from Tuesday - Thursday from 9 am to 12 pm. Individual diabetes education and medical nutrition therapy for diabetes are held Monday - Thursday from 9 am - 4pm. Referrals are required from a healthcare provider. Most insurances are accepted and cover the cost of this service. Please call to make an appointment.

In addition to consultative service, the Division has weekly clinics and office hours which provide specialized care for patients with endocrine disorders, such as diabetes, thyroid, adrenal, metabolic bone, calcium, lipid, pituitary, reproductive and growth. In addition, the Division has an American Diabetes Association recognized diabetes education program taught by certified diabetes educators for patients requiring diabetes self-management skills.

The opportunity to participate in a clinical trial can occur at any MedStar Health facility depending on the current trials that are open to enrollment at each site.

Pre-Diabetes Program

Pre-Diabetes Program

A program designed for those diagnosed with pre-diabetes who would like to work toward preventing diabetes.

Three months of four nutrition classes and personalized exercise. The nutrition classes are presented by Registered Dietitians and are held at ThedaCare facilities. The participant also has the opportunity to attend the YMCA of Fox Cities for a variety of exercise options (classes, personalized fitness program, swimming, trails, etc.) throughout these three months.

Cost: nutrition classes are $26 per couple; YMCA grants and scholarships are available.

Diagnosis of pre-diabetes required: Fasting glucose 100-125 or oral glucose tolerance test 140-199; or A1C 6-6.4%.

Locations Vary: Appleton Medical Center on Mondays 5:30 p.m. to 6:30 p.m.; Wednesdays 1:30-2:30 p.m.
ThedaClark Hospital on Wednesdays 5:30-6:30 p.m.
