Centra Health's Diabetes program is designed to promote diabetes self-management. Patients need to provide a physician referral so we'll have information about their need.

We provide diabetes education to patients, family members and caretakers, and we offer inpatient as well as outpatient education. Through our program, people recently diagnosed with diabetes learn survival skills including:

- a basic intro to the disease process
- foot care eye and dental health
- meal planning and food label reading
- guidelines for sick day management
- medication information (oral or insulin)
- self-monitoring guidelines instructions on use of the glucose meter
- exercise to promote better control of the disease and improve well-being
- managing complications traveling skills
- intensive insulin management

Understanding New Treatments For Diabetes
Wednesday, March 2 • 10 a.m. to Noon

Diabetes medications have continued to evolve over the past several years, and new
therapies are anticipated in the market over the next year. Alan Kauppi, M.D.,
endocrinologist, will discuss these treatments and medications. He will help you
better understand how some medications are beneficial for some people with
diabetes but not for everybody. Dr. Kauppi also will talk about treatments that will
become available soon.

Diabetes Education for those newly diagnosed or previously diagnosed with diabetes.

We are an American Diabetes Association approved diabetes self-management education program and provide diabetes self-management education as an interactive, ongoing process that involves the person with diabetes and the educator/diabetes coach. The intervention aims to achieve optimal health status, better quality of life and reduce the need for costly health care. We focus on seven things patients can do that are essential for improved health status and greater quality of life: healthy eating, being active, monitoring of blood sugars, taking medication (if needed), problem solving, healthy coping and reducing risks. Our diabetes educators give people with diabetes the knowledge, skills and tools they need to successfully manage their diabetes and avoid many of the complications associated with the disease.

We provide our services Monday through Thursday, from 8:30 a.m. to 4:00 p.m.

Our two diabetes educators are certified diabetes educators with a Master's Degrees; one a Registered Nurse and one a Registered Dietitian. Our services are covered by Medicare and most commercial insurance plans. However, individuals should check with their specific plan about coverage and if a referral is required.

The endocrine clinic provides affordable specialty care to patients suffering from endocrine-related conditions.

Mobile Medical Care, Inc. (MobileMed) is a nonprofit 501(c)(3) organization whose mission is to provide quality medical care to uninsured, low-income and working poor individuals in Montgomery County. We operate culturally-sensitive clinics in our fully-equipped mobile medical units and in community buildings located throughout the County. MobileMed’s program is unique in that it is committed to a community-based service delivery model that encourages collaboration, increases access to health care and removes barriers through the delivery of linguistically and culturally appropriate services. MobileMed continues to work toward improving the health of the County’s poorest, most isolated and vulnerable residents. In 2010, we provided much-needed health care services to over 6,100 individuals.

The endocrine clinic provides affordable specialty care to patients suffering from endocrine-related conditions. Individuals must be registered as a Mobile Med (MM) patient and be referred into the program by a MM provider.
