DC Chronic Care Coalition

The Chronic Care Coalition of Washington, DC is the supporter of the first well populate region for DiabetesLocal.

 

Virginia Diabetes Council

The VDC is a non-profit Council focused on improving the lives of Virginians affected by diabetes. The Virginia Diabetes Council provides a forum for communication, collaboration and action among stakeholders to reduce the burden of diabetes in Virginia.

 

Alaska Diabetes Control Program

The Alaska Diabetes Control Program is a part of the CDC’s Diabetes Prevention and Control initiative.  Alaska was the first region to migrate their published Diabetes Resource Guide to DiabetesLocal.

 

Wisconsin Diabetes Control Program

The Wisconsin Diabetes Control Program is a part of the CDC’s Diabetes Prevention and Control initiative.  Wisconsin has populated DiabetesLocal with resource through an initiative to engage appropriate organizations.

TOPS

Take Off Pounds Sensibly, TOPS Club, Inc. is the original, nonprofit, weight-loss support and wellness education organization.  TOPS' Mission is to support our members as they take off and keep off pounds sensibly.  TOPS was the first international organization to offer support to DiabetesLocal through complete data of their member locations.

 

Nevada Diabetes Association

The Nevada Diabetes Association is a 501(c)(3) non-profit independent diabetes association and is not affiliated with any national diabetes organization.  The mission is to improve the lives of children and adults affected by diabetes through prevention, education and service.

 

ADA Recognized Education Programs

The American Diabetes Association (ADA) sets standards and reviews education programs across the United States recognizing those programs that meet their high standards.  The ADA has provided a complete list of these recognized programs to allow DiabetesLocal users to easily identify quality diabetes education in their community.

 

Hillcrest Medical Center

The Hillcrest Medical Center is located in Tulsa, Oklahoma.  The Center for Diabetes Management strives to provide quality support to their patients.  In an effort to enhance the current services, the Center has teamed with DiabetesLocal to provide a continuously updated, living guide of locally accessible resources and support.
