Diabetes Case Management Services

Physicians Plus offers free Diabetes Case Management services to its members. Do you need help reaching your Diabetes Self-Management goals?

Diabetes Case Management services are free and available to eligible members who have uncontrolled diabetes and Physicians Plus Insurance (covering southcentral Wisconsin). You will work closely with a Diabetes Nurse Case Manager over the phone who will help you set and work towards goals aimed at improving your diabetes and overall health.

The Diabetes Case Management program is individualized based on your needs and can last anywhere from three to 12 months. Your Diabetes Nurse Case Manager will help you identify areas in which you struggle with your Diabetes Self-Management and help you set realistic goals to overcome those barriers. They will keep you up to date on the latest recommendations in Diabetes Self-Management and provide you with educational materials when needed.

Wisconsin Diabetes Prevention and Control Program

The Wisconsin Diabetes Prevention and Control Program (DPCP) is dedicated to improving the health of people at risk for or with diabetes.

Much of the health and economic burden of diabetes can be averted through known prevention measures. The Wisconsin Diabetes Prevention and Control Program (DPCP) is dedicated to improving the health of people at risk for or with diabetes. We rely on our strong partnerships in the development, distribution, and implementation of resources.

Publications for people with diabetes include:
- Self-Care Information and Record Booklet
- Personal Care Record (wallet card)
- Blood Sugar Log Booklet
- Students with Diabetes: A Resource Guide for Wisconsin Schools and Families

Any type of tobacco is harmful to the health of all people, including those with diabetes and pre-diabetes. Smoking raises blood glucose, cholesterol, and blood pressure. Tobacco cessation lowers the risk for heart attack, stroke, nerve disease, kidney disease, and oral disease related to diabetes.
