Hayward Memorial Hospital Outpatient Diabetes Education Program

American Diabetes Association recognized program providing individuals and families comprehensive diabetes education related to self-management of diabetes.

Diabetes Self- Management Education at Hayward Area Memorial Hospital is provided by a Registered Nurse/Certified Diabetic Educator and a Registered Dietician/Certified Diabetic Educator.
The Diabetes Self-Management Education Program provides on-going comprehensive diabetes education to each participant. The initial education is 1:1 in the clinic office and family members are encouraged to attend, a 2 week follow up and a 3 month follow up visit are scheduled at the first initial appointment, thereafter through-out the first year, education sessions are scheduled pr individual needs.
On-going contact with your educator is very important to maintain control of diabetes, after the individual recieves comprehensive education the first year then appointments are scheduled bi-annually or per individual needs.

Monthly educational/support group meetings are provided at no charge to the public, the first Tuesday of every month at 3:00 pm in the hospital conference room.

Primary Care Physicians will identify individuals who can benefit from our service, the physician will send a patient referral to our office, upon receipt of this referral the individual will be contacted and an initial appointment for education will be arranged.

Most major insurance companies, Medicaid and Medicare provide coverage for diabetes education. To verify your individual coverage please call the customer service number on your insurance card.
