Diabetes Outpatient Education Program

Adult Diabetes Self-Management Education for Type 1 and
Type 2, including Insulin Pump Self-Management Education

Free Diabetes Support Group meets monthly March-November

American Diabetes Association Recognized Program since 2000. Two full time RN Certified Diabetes Educators and two part time Registered Dietitians provide staffing for the program. We provide comprehensive diabetes self-management education for adults with type 1 or type 2, diabetes in pregnancy and gestational diabetes mellitus.

Education is provided in group classes or individual education appointments.
Insulin pump self-management education is also available.

There is a fee for these services and medical insurance, Medicare or Medicaid is billed. Insulin instruction and insulin adjustment is one of our services but the referring physician must have privileges at Aurora Memorial Hospital of Burlington. The same stipulation applies for patients wanting insulin pump self-management education.
