Cell Phone Assisted Self-Care Diabetes Management Program

Provide Intensive and Lite cell phone programs for patients with diabetes.

This is an exciting 12-month cell phone based intervention program, that aims to improve self-management behaviors in patients with diabetes in an effort to reduce emergency department visits and hospitalization.
The program builds on the existing model which consists of clinic-based healthcare providers and case managers and adds a cell-phone based application to ensure blood sugar control through self-monitoring, real-time feedback, coaching and support. The phone or computer application will also provide summary reports to health providers and case mangers in order to help them make better and faster treatment decisions.

Comprehensive diabetes education for all kinds of diabetes

Anyone 12 years of age and older with any kind of diabetes (Type 1, Type 2 and gestational) whether newly diagnosed or in need of a review would benefit. Most insurances are accepted including Medicare and Medical Assistance. We have an endocrinologist and experienced certified diabetes educators (RN and RDs) who provide individualized care. All aspects of diabetes care are covered such as healthy eating, exercise, medications, monitoring and healthy coping.Leading edge services include insulin pattern management and adjustment, insulin pump therapy and real time continuous glucose monitoring. We offer regular business hours with appointments available within 1-2 business days, provide follow-up to your physician (referral needed) and are handicapped accessible.

This program has been approved by the American Diabetes Association as meeting the National Standards for Diabetes Self-Management Education.

In order to self-manage diabetes effectively it is important to engage in diabetes self-management education programs. These programs cover a variety of helpful content - nutrition management, the importance of physical activity, medication management, glucose monitoring, and the psychosocial issues related to diabetes. Diabetes self-management education is a critical element of care for all people with diabetes and will help you live successfully with your diabetes. Ask your healthcare provider for a referral to a diabetes education program.

Support Groups
Insulin Pump Support Group
The focus of this group is on using insulin pumps to manage glucose, however the meetings are helpful to anyone with diabetes who wants to meet with other diabetics and learn more about the challenges of managing diabetes, meetings are free and open to the public. The group meets every other month on the 4th Tuesday from 7pm-8pm. Call beforehand to confirm meeting time and location.

Ask the Educator
The focus is on living a full and healthy life with diabetes. The group offers support, advice and the latest health information. This meeting is held the third Thursday of each month from 2pm-3pm. Call beforehand to confirm meeting time and location.

We will do our best to serve you. Rappahannock Neurology Specialists is a comprehensive and multispecialty neurology and sleep medicine clinic, based in Fredericksburg, Virginia. We are a highly trained group of four neurologists who can provide the communities in central Virginia and great Washington Metropolitan with academic level neurological care. We practice evidence-based medicine in the treatment and advances of neurological diseases and engage in clinical research (see our ongoing trials). More importantly, we believe in an individualized, compassionate and personable approach to our patient care.

In addition to our outpatient practice, we are part of the physician group that evaluates and treats admitted patients at Mary Washington Hospital and Stafford Hospital. We cover the hospital 24/7. We are all board certified neurologists who are either fellowship trained or have advanced specialty training. We work closely with other disciplines and experts that include neurosurgery, neuroradiology, physiatry (rehabilitation), neuropsychology and social workers. Although we are primarily adult neurologists, we are experienced in the evaluation and treatment of patients who are 13 years and older. We welcome new patients to our outpatient clinic.
