Nutrition education
Individual appointments are available with a registered, licensed dietitian. A physician referral with a nutrition related diagnosis is required. All insurances are accepted, but clients are encouraged to contact their insurance company to confirm coverage before scheduling. Our facility is handicapped accessible, and parking is free.
Patients with an established Unity Health Care primary care provider can receive services.
Clinic Hours: Monday and Wednesday 8:30 – 8:00 pm (Extended Hours 5:00 -8:00 pm) Tuesday, Thursday and Friday 8:30 – 5:00 pm Saturday 8:00 – 12:00 pm

Registered Dietititian and Certified Diabetes Educator, Lise Gloede has a successful practice helping to provide Medical Nutrition Therapy for Diabetes - Type 1, 2, Gestational Diabetes, Pre-diabetes, Cardiovascular risk reduction, metabolic syndrome, weight management and many other chronic conditions. Many insurances accepted. Visits by appointment only. Hours Monday 8-5, Tuesday 1-5, Friday 8-5
Case Management
Assists seniors to live independently in a private residence, using physical and occupational therapy devices, special equipment and specialized assistance and to cope with other requirements for successful independent living. These include the ability to travel alone, live within one's personal income, maintain acceptable grooming and appearance, cope with legal, family, or social problems and maintain health through self-care and the effective use of medical services.
