The program - which is taught by registered nurses and dietitians who are certified diabetic educators - helps those with diabetes understand the disease and teaches them how to manage it. Participants receive assistance with glucose monitors, can get nutritional advice with meal planning and learn about the treatment options that are available.
Class series are offered on an ongoing basis mornings and evenings. A prescription for diabetes education and nutritional counseling from your physician is required. The fee for this service is often covered by insurance and Medicare.

All sessions are held at The Center for Healthy Living, 198 Prospect Street, Lakewood, on the Kimball Medical Center campus.

In addition to the series of educational classes, Kimball Medical Center also offers free monthly glucose screenings and a free monthly support group aimed at ensuring that patients not only understand their condition, but play a role in the control of their illness.

The Diabetes Management Team at SOCH

The Diabetes Management Team at SOCH includes certified diabetes educators, registered nurses, dietitians, pharmacists, podiatrists and fitness instructors who specialize in diabetes self-management training. This multi-faceted team of professionals works closely with physicians and specialists to help participants manage their diabetes safely and effectively.
Participants of the Diabetes Self-Management Program meet team members in a one-on-one setting to assess their needs and design a customized educational program. Services include instructions on how to safely administer insulin, the proper care and use of a self-blood sugar monitor and more. Team members take time to provide hands-on practice to ensure participants are comfortable with their medication regimen and proficient in self-blood sugar testing and all aspects of diabetes care. We are an American Diabetes Association approved diabetes self-management education program and provide diabetes self-management education as an interactive, ongoing process that involves the person with diabetes and the educator/diabetes coach. The intervention aims to achieve optimal health status, better quality of life and reduce the need for costly health care. We focus on seven things patients can do that are essential for improved health status and greater quality of life: healthy eating, being active, monitoring of blood sugars, taking medication (if needed), problem solving, healthy coping and reducing risks. Our diabetes educators give people with diabetes the knowledge, skills and tools they need to successfully manage their diabetes and avoid many of the complications associated with the disease.

We provide our services Monday through Thursday, from 8:30 a.m. to 4:00 p.m.

Our two diabetes educators are certified diabetes educators with a Master's Degrees; one a Registered Nurse and one a Registered Dietitian. Our services are covered by Medicare and most commercial insurance plans. However, individuals should check with their specific plan about coverage and if a referral is required.
